  • All children returning on the van must be picked up by 4:00 at CDC (considered regular fee) no extended hrs. avail at cdc
  • There will be a $15 late fee for any tuition paid after the due date. Check the box above for carpool information.
  • The deposit will be deducted from your last session’s tuition. Your deposit will be forfeited if you cancel any sessions.
  • Please make a copy of this form for your tuition deadlines, or ask for a copy from Kyle or Kelly
